Your journey with Comfort Care Services begins with an initial referral from your Treating Specialist, General Practitioner, Veteran’s Liaison Officer, or any other DVA provider.

Step 2: Comprehensive Assessment

Once we receive the referral, a Registered Nurse will visit your home to conduct a thorough assessment. This includes evaluating your clinical needs, understanding your personal goals, and developing a tailored care plan just for you.

Step 3: Personalized Care Coordination

After the assessment, the details will be shared with our rostering officers, who will carefully select compatible staff to meet your specific needs.

Step 4: Collaborative Care

Our team works closely with various allied health professionals, including Occupational Therapists, Physiotherapists, Dieticians, Podiatrists, Speech Pathologists, and others. This collaborative approach ensures that you receive comprehensive and compassionate care that addresses all aspects of your health.
